I got your response and I crawled in to the DMV but they wouldn’t accept it. It didn’t say whether I was capable of driving! I want my money back as I had to go back to my dr and get a new form .

This couldn't have been a better, or an easier experience. I submitted my info on a Tuesday. Within 2 hours I had a call scheduled with a doctor. From there the Dr and I spoke about my condition, and he approved me. We were on the phone 15 min tops. I received my my signed form on Saturday. I would highly recommend.

Dr.Sasso was excellent, but it took 6 days in the mail system. Might consider using a faster method!

I filled out the application and submitted my information within 8hours of submitted documents I was contacted by a Physician. Within 48 hours my forms came in the mail. I was really pleased with the professionalism of the Physician and how quickly everything was handled. I will definitely recommend this site.
