Fill out basic questions and answer the questioner pertaining to your medical condition, of course you have to have supporting documentation to give to the Dr who will review your case. Sent my paperwork and payment and in less than 2 days I was at the DMV getting my placard. Thank you Divanmed for helping out people with disabilities, process was 5 stars
