Piece of cake. Essentially, transcribed my Veterans Affairs disability list onto the website questionnaire. Dr Handicap reviewed it, made a determination, filled out the Oklahoma disability placard application form and sent the nearly complete form back to me. I had to choose the number of placards requested, sign it and mail it to the state. Piece of cake!

Easy and straightforward; saved me a lot of time and hassle. The doctor was helpful and responsive. And I went to the DMV as soon as I received the forms (which came very quickly), and got my handicapped license plate and placard with no problem. I deeply appreciate the assistance!

It was a very easy and quick process! Although I need a placard, I was scared to ask my rheumatologist about initiating the paperwork--due to the large health care institution she practices in and certain biases. Luckily, this medical group helped me out and I am forever grateful! My placard is enroute to me as I type this.
I paid you $159 on August 11. You still have not completed the job that you committed to which was to submit my paperwork online to the Polk County DMV. I have emailed, text and called many times to no avail. All I ever get is I will pass it on to the appropriate person. I would not recommend your company unless you correct this immediately and let me know that it is completed on your end so that I may check with the DMV.

Fast and painless. Was worried about the stigma of asking my own doctors. My doctor used their notes, expertly wrote notes summarizing their findings, and came to the same conclusion. Sent out the script the next day and the DMV paperwork. This place is amazing if you feel uncomfortable asking ‘that’ question from your doctors, especially when dealing with a lot of stuff after a bad motor vehicle accident.

Easy and trustworthy experience. The correct paperwork was sent to me and I now have my permit. There’s nothing sketchy about this service. If you have a legitimate medical history and disability you will have zero issues. I had the paperwork and hangtag within 2 business weeks due to a delay in shipping. Yes I am a real person, no I was not paid to post. Yes I also came to read up on these comments to see if this was legit or not. I applied August 1st. Approved August 3rd with a USPS tracking number. Estimated ship date was the 6th. It didn't arrive to USPS until the 12th I believe. Arrived in my mailbox on the 16th. Accepted by the DMV when I went in the next day. About 17 days total. They answered all my calls ( do not press 0. Just wait and it will go through ) They responded to all my emails, sometimes a few hours later. They may be pricey to some but it works. At least my experience. Depending on what you submit medically, will determine how fast they process it or if they approve it. I recommend and this is just my experience.
