The process was quick and easy, Everything was made simple and the instructions were straightforward. Communication was excellent.

They took weeks to get this completed First they said oh we hired new dr you are 25 on the list now 8 while I tried multiple times to text the system with zero response I had to call multiple times and was basically lied to saying I was 8th on the list and still waiting until I spoke to the actual manager who said no there in fact is still no hired dr. Then I waited an entire week after the dr was hired and I had to keep calling daily to make sure it was processed. In the meantime the actual web site had issues and kicked deleted me as a client and the person under the manager told me I did it when I didn't. It restarted everying and was awful. When I did finally get approved - it I never got an original signature so HAD to upload it on the DMV website and am STILL WAITING to get the placard from the DMV so its well over a month since I started this process and still dont have a placard NOT ACCEPTABLE that I was lied to by Sam and she was very short with me. Jessica was nicer but still I was not told the truth by either about NO DOCTOR SITUATION and then having them Sam blame me that I in fact deleted my order when in fact the website was having huge issues as well.

After providing my medical file to Divanmed, I received the necessary medical forms for a handicap permit. Florida DMV accepted the completed forms without question and my parking issues have been resolved. Thank you, Divanmed.

I uploaded my documents, paid the $159 and never heard from them. So I contacted my bank for a refund. 8 days later I was getting the mail in and there was a priority mail envelope in the box. It was my signed physicians form. I was so happy! I would give them 5 stars but I’m only giving them 4 because of the communication break down.

Amazing service! Honestly was skeptical at first but decided to give it a shot. As noted when submitting it takes on average appr 2 weeks. Mine was complete in less than 2 weeks :) ! The initial day I submitted all my documents (diagnosis, treatment summaries, med list, and videos) and was assigned a physician that practices in my state. The following week I received the approval with notes from my doctor. Approximately 3-5 business days later I received my signed documents that were applicable to my state. Love it! I’d highly recommend if you’re reading this! Great service!

I dont have my paperwork but yet you send a "how did I do" email. Maybe wait until your package arrives and see if it worked for me instead of asking for me to rate a service that hasn't been delivered.

I felt like the doctor didn’t believe me, when the first one gave me my placard without question because of my condition. And this time I have to mail the paperwork to the disability office, but I didn’t have to last time. It was way more of a hassle and I don’t think it’s worth what I paid for it.
