Being 80 yrs old... its difficult to respond electronically... lots of forms and information are needed in order to complete the application. Excellent... I repeat... Excellent service and patience was given by Dr Nakamura. He made me feel very comfortable throughout the ordeal. He also responded very quickly and explained everything very clearly.

I am handicapped and have trouble getting out and they were able to get me an Approval for a handicapped mirror tag for handicapped parking. The form was filled out and signed by the doctor for my state. All I had to do was sign my part and was good to go. Sent form to my tag office and had no issues getting my tag.
