I was dealing with severe and chronic left heel pain for years - been treated, and seen everyone but nothing helps. Never thought to seek a disabled placard card, even my PCP didn't mention it despite my difficulty walking. Sent all my records in here, took a video of me walking, provided additional information as requested, and got approved. All in all, very professional and expedient.

Received my disability cards…the doctor was very helpful. The doctor wrote a prescription for the placard and plates …but I was only given the placard…and I’m very grateful for that.thanks doc.

Horrible.....just do it yourself they don't help with anything! I'm sure no one will see this as they will delete it I'm sure!

The process was very easy and straightforward. Completed online questionnaire, uploaded supporting documentation and was contacted by a doctor who assessed my condition and granted my approval the same day. Received my placard application in the mail about 10 days later. Would have been quicker if the bad storms hadn’t delayed the delivery process.

I am not going to lie, I was really hesitant to use this service because of the negative reviews; however, my experience was GREAT! I also reached out to customer service for clarification and they were responsive both times. It goes without saying...always go with your gut. I did and so very thankful for Dr. Handicap and my new placard.

Basic healthcare has been a very difficult resource to obtain as I am getting older. My providers I have had for many years have constantly discriminated me against my age for critical health problems I have had and trying to obtain any accommodations from them were never successful. The provider I was given by Divanmed respected me and made me feel 'seen' and reviewed my request with great care. I am truly grateful.
