Terrible service
Terrible service. Paid over $150.00 never let speak with a doctor in four weeks.

Form has been very helpful on my weight loss journey. I truly appreciate the support of the community and staff. It helps knowing you are not alone or the only one who is struggling with weight gain.

The medical care team keeps getting switched and there is no timely follow up

Form health has been the easiest and best thing to use. I have been using it for almost a year and the support team, doctors and dietitians have all been amazing. Their service has always been top notch.
Great staff and wonderful
My insurance company required we use Forum Health to maintain my insurance coverage for my weight loss medication. However, my insurance company has since decided to NOT continue coverage for the medication I have been on for a year unless my doctor writes an exception. When I brought this up during my zoom visit I was told I will have to go on a different medication. The one that is recommended by my insurance company did not work for me. I was hoping my doctor from Forum Health would have been more of an advocate and fight for what is best for me which is staying on my current medication versus making her job easy and just putting me on whatever my insurance will cover. She didn’t even attempt to address the appeal process with my insurance company.
It has been very nice to feel supported not only by the medical doctor, but also the nutricionist. I have been able to make healthy changes.
Chantel has been absolutely amazing! She takes the time to answer all my questions, she follows up after every visit and again during the week to check on how I’m doing. She is also incredibly encouraging and kind!

Form is so easy to log meals, weigh ins and book appointments there’s no excuses to not do it! But the app provides so much more than that! I know that all the support I could ever need is just a text away and that my team is always there for me. Paired with the Bluetooth scale they provide, I can easily track my weight and my dietician is always looking up tips, suggestions, recipes and more specifically related to my personal needs and goals. My assigned doctor has access to all my health records and I know she is looking after all my health concerns and making decisions that are best for me. I look forward to my monthly remote check-ins to keep me on track. This is by far the best program out there!

The combination of medication, a very knowledgeable dietician, and a great doctor for advice have provided me with the most success I’ve ever had to manage my weight and improve my health.

Form has great HCPs- they are capable, courteous, and timely. The administrative side of Form is poor at best- too many delays, missed communications, and lack of follow-through.
