Bait and switch
Bait and switch. They say they bill insurance but they don't and they refuse to submit, instead they want you to pay a monthly membership fee plus doctor visits. Insurance confirmed they never submitted any claims despite the billing department saying they did submit and got denied. Edit to add: You can't get a reply from anyone beyond a generic this is how it is response. My husband has emailed and I sent an email. There wasn't even an attempt from Form to transition his care as stated in their Faqs. Just a solid you're cut off if you don't give us a monthly membership fee and pay for doctors visits. Again, nothing was billed to our insurance yet they claim it was all denied. I stand by the bait and switch allegations.
