When I inquired to my doctor my interest in weight loss, he referred me to bariatric Drs. To my surprise, it would take 7 months to get an appointment. While I was at my arthritis Dr., she told me about Form. I immediately went online to apply. From that moment on, I have been motivated, encouraged, supported and best of all, I’ve lost about 20lbs.
