Knowledgeable and caring staff!
I have been a FORM Health patient for approximately 9 months. I was hesitant at first to utilize telehealth services as I was feeling it would be impersonal and the quality of care may not be what I have experienced with face-to-face care; I could not have been more wrong. The FORM Health staff are extraordinarily knowledgeable and personable. My doctor always explains everything in detail and my nutritionist is phenomenal. The care coordinator is always responsive and helpful. I would encourage anyone thinking about joining FORM Health to absolutely take the leap. They will not be disappointed!
