I am 100% pleased with Form.
I am 100% pleased with Form. I feel I’ve received personalized care by my weight loss physician and dietician. They have both been understanding of my lifestyle and were supportive when I took a pause due to surgery. Ideally, my insurance would have covered weight loss treatment, but I can’t imagine doing any better through an in-person program.
